I have an S90 ES is connected to to a Mac Pro (early 2009) via a USB cable to a USB hub (the computer is in a closet, too far for the USB cable to reach).
It frequently loses contact with the Mac. I've mainly been working with MainStage 3 and Sibelius 8 (latest versions of each) recently, but I think it happens with my DAWs as well (Digital Performer 9.13, Pro Tools 12.8).
Yamaha USB MIDI driver is 1.3.2.
This is not new - it's been going on for years. Usually I just unplug the USB cable from the S90 ES and plug it back in, and it works again for a few minutes. I have to do this several times each work session.
However, today that's not solving the problem, as I'm trying to work in Sibelius.
Wanted to check the S90ES firmware version, but I can't find any info online or in the manual about how to do that. In the first few years I owned it, I always kept up, so I suspect it's the last version...
All applications are up to date.
Mac OS is 10.11.6. 40GB RAM.

All of your issues are known by you. You are proving why it is highly recommended to NOT use a hub. I'm surprised you are able to get it to work some of the time... you seem to know this is a problem. You need to work on a solution that eliminates this hub (its great for non-timing sensenitive devices like your printer...). Give yourself a break, move your studio and keyboard closer together... you asked for suggestions, that's my suggestion (eliminate that hub, at least you'll be able to know if it is indeed the cause - right now it is the leading suspect! 🙂
Moving the computer into the room is not an option, as I'm highly sensitive to the fan noise, especially when editing/mixing/mastering.
However, I bought a 15-foot USB 2.0 cable, which was long enough to reach from the S90ES to the computer. So far, the direct connection seems to have resolved the problem.
