I've had this happen once when trying to save a set change by pressing the enter button and accidentally rolling it while pressing it. It rolled to the adjacent voice and overwrote it instantly. Guess we need that "Are you sure?" prompting showing what's about to happen. Or change the way we store a change maybe by pressing store twice instead of the dial. I think there's also an undo command which you have to hunt for.
